• Web based easy to use 8 step online tool that covers all standard event types, heads up feature for possibly reportable events, report writer with standard reports ready to use
• Knowledge Builder differentiates between near-miss events, no-harm incidents, incidents with harm, and sentinel events as well as claims and lawsuits
• Unlike most other systems, Knowledge Builder collects text and coded data on both contributing and intervening factors

Why it is needed:

• Existing systems count the "what where and who". Knowledge Builder goes more than one step beyond to measure contributing and intervening factors. Reports for 3 levels provides information from board to unit level
• Combination of text and coded field leads staff to consider factors. Asking the staff opinion encourages reporting
